Assignment of Benefits
Patients assign payment to JVL Dental Creations P.A. / Sunset Smile Creations; of all dental benefits applicable and otherwise payable to the patient by its insurance carrier, including HMO, PPO or any other third party payor, for services rendered at Sunset Smile Creations. Patients understand that they are financially responsible to Sunset Smile Creations, for charges not covered by this assignment or any and all charges which the carrier declines to pay.
